THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our duties

We are required by law to maintain the privacy and security of your protected health information, to give you this notice of our legal duties and privacy practices, to follow the terms of the notice currently in effect, and to notify you if a breach occurs that may have compromised the privacy or security of your information.

How we may use and disclose your health information

Treatment. To provide, coordinate and manage your care, including sharing information with your clinician, pharmacy and laboratory.
Payment. To bill and collect payment for the care you receive.
Health care operations. For quality assessment, clinical review, training and business management.
As required by law, including public health reporting, reporting adverse events to the FDA, and responding to lawful requests from health oversight agencies, courts or law enforcement.
To avert a serious threat to health or safety.

Other uses and disclosures, including most uses of psychotherapy notes, marketing, and any sale of your information, require your written authorisation, which you may revoke in writing at any time.

Your rights

Inspect and copy your medical record, generally within 30 days of your request.
Request an amendment if you believe information is incorrect or incomplete.
Request an accounting of certain disclosures we have made.
Request restrictions on uses and disclosures. If you pay in full out of pocket for a service, you may restrict disclosure of that service to a health plan.
Request confidential communications by a particular method or at a particular address.
Receive a paper copy of this notice on request.
Be notified of a breach of unsecured protected health information.

You will not be retaliated against for filing a complaint.

Changes to this notice

We may change this notice and apply the change to information we already hold. The current notice is always posted here with its effective date.
